I don't know if any other hotels do this but I have asked for one in the past for our son and we got a birthday button with his name on it and a nice signed photo of Mickey for him along with some stickers, disney trading cards and some balloon's it was so very magical for him.

We have also received the sticker but that was it. My friend Mickeynewe's daugher had a birthday button on and she received free desert and a pin while she was wearing her button. My son was not that lucky but every CM said Happy Birthday and that sure made him feel extra special. :0)

Ds's bday was over spring break so we got him a bday sticker at City Hall-missed the phone call for some reason. Just having the sticker on means almost every cast member said Happy Birthday and greeted him by name! It really makes the day special for some but our shy guy just kept blushing the whole day-lol!
We made ressies for the bday party at the Plaza with "Pat E. Cake" for a small amount of $, can't remember maybe $10. Each of us got a bday cake to decorate and they sing and then some "Special Guests" come and parade around and take photos.
Since it was Spring Break, I was estatic not to have to wait in the toon town lines for this "Special" picture. Outside the Plaza, right at the entrance where you check-in, they have a cart and you can buy other bday items like pins, buckets, etc. but remember to look and add on BEFORE you go in if you want those things.
My kids loved it although dd who was not celebrating and 5, was so sad and even had a tear in her eye because they wouldn't let her walk around with the "real" bday kids. Almost ruined it for us so be prepared if you have other kids for that part.
Goofy's can do a "real" party for a nominal fee ($5?) everyone gets a party favor of some kind and they sing and bring cake. You can do add-ons like the buckets here too.
We are taking her to BB and ordered a bday bucket there. I think it's $20 extra.
The only other "free" tangible thing that I know of is getting a pack of tortillas at the tortilla factory instead of just one. Even that is not guaranteed though.
You can ask for other special things like riding up front in the monorail, wheelhouse on the Mark Twain, etc. but you can always ask for those things, it's just you "might" be more likely to get them if it's your bday.
Hopefully, others will post some more things in time for my dd's bday celebrations!
I'm also going for my bday in Jan and looking for extra things to make it extra special too.

I have a birthday page you might find useful. The only thing out of date really is that the merchandise cart outside Plaza Inn doesn't have all that stuff anymore. They only have a few items. An ODV CM heard us talking and told us that she usually has some and felt badly that she didn't. That surprised us as we thought only City Hall had them. DD ended up getting me a sticker from a CM on the dock at the Jungle Cruise. They wrote my name on it and "50" in small print :)

Just FYI if you blow by City Hall like we did or take the monorail in.
