My guess is that this QR code on the guide map takes you to a page where you can download and install an applet of some sort, and that applet works with the camera in the cellphone to recognize these plaques if you take a picture of them and that something happens if you find all of them, but I have no way of knowing since I don't have a Japanese cellphone and can't read the webpage. Does anyone know what the real deal is?

Those have a "Kimmunicator" (basically a cell phone) that they give you and you follow clues to specific locations throughout the World Showcase and trigger small events (things lighting up or items moving in store displays) that lead you on your way to defeating some evil villain. I have no idea if they are doing something similar (but Monsters Inc themed) in TDL's TL, but that seems like the type of thing that might be really popular in Japan, especially with the right character tie-in.

Certainly, the QR Code stuff is universally implemented on Japanese cellphones, but what I don't know is if the downloadable applets and their ability to access the camera is universal enough that they could rely on it working on most phones.
