Do I send the package to his actual address, or to the PO Box? He is out of the country currently so I can't aks him.

Doesn't the US Post Office deliver directly to the homes?

To be on the safe side, I'd send it to the PO Box because they'll leave notice in it for your friend to go to the counter to pick up the package.

Thanks for the comment. I've already sent the package. I put the PO Box number and the address on the box. He received it.

Also, some people choose to maintain a box because of issues with receiving their mail (theft, they move/travel a lot, or because their address gets confused for somewhere else frequently).

If you list both a street address and PO Box, the USPS delivers the address listed on the lowest lines. (This is also true if you put two street addresses.)

    John P. Doe
    123 Main Street
    Anytown, CA 99999
    PO Box 4683
    Somwhere, CA 99998

For example, would go to the PO box.
