You have to show your Disney Visa and go downstairs to have your picture taken. It's usually Chip and Dale or Mickey and Pluto as the characters. It's only in the morning when they first open for the first couple of hours I believe. You can check for sure at the information booth just in the entrance to the park.

You have to pick them after a certain time (maybe 3PM?), but before your time window closes (maybe 48 hours?). They'll give you a claim check and tell you all about it when you have your pics taken.

I have been using the card gingerly, waiting for the axe to fall. Well, it worked consistently for a month but today THEY DID IT AGAIN!!!! I HATE THAT CARD!!!!!
